ZOZOFIT User Survey Incentive Program – Terms & Conditions

1.  Eligibility

Participation in this survey incentive program (the “Program”) is limited to individual ZOZOFIT users who:

• have a registered ZOZOFIT account,
• have completed two or more body scans in the ZOZOFIT app, and
• receive or access an invitation to participate in the survey from ZOZOFIT.
  

Businesses, agencies, bots, or other non-individual participants are not eligible to participate.

2. Qualified Completion

A “Qualified Completion” is defined as a survey response submitted by an eligible user who:

• completes the survey in full,
• provides responses that are reasonably complete and valid, and
• complies with these Terms & Conditions and any applicable instructions provided by ZOZOFIT.
  

Incomplete, duplicate, fraudulent, automated, or otherwise invalid submissions do not qualify.

3. Reward

Each eligible user who completes a Qualified Completion will receive one (1) USD $10 Amazon Gift Card.

Limit one (1) reward per eligible user under this Program, regardless of the number of survey attempts or submissions.

4. Reward Fulfillment

Rewards will be fulfilled electronically by email to the address associated with the participant’s ZOZOFIT account or another email address validly provided for reward delivery, as determined by ZOZOFIT.

Rewards may be delivered directly by ZOZOFIT or by a third-party reward fulfillment provider engaged by ZOZOFIT on its behalf.

Reward fulfillment may take place only after ZOZOFIT verifies eligibility and Qualified Completion status. Delivery timing may vary.

5. Reward Restrictions

• Rewards are fulfilled as Amazon Gift Cards only.
• No cash, alternative gift cards, substitutions, or other forms of payment will be offered.
• Rewards cannot be transferred, exchanged, or redeemed for cash, except where required by law.
• Participants are responsible for ensuring that their email address is valid and able to receive reward-related emails, including emails sent by a third-party fulfillment provider on ZOZOFIT’s behalf.
• ZOZOFIT is not responsible for delivery delays, email filtering, invalid email addresses, or other issues outside its reasonable control that may affect reward receipt.
  

6. Duplicate, Fraudulent, or Improper Participation

Multiple submissions by the same person do not count as additional Qualified Completions.

If ZOZOFIT determines, in its sole discretion, that multiple submissions, accounts, or email addresses were used by the same individual, or that any submission was generated through fraud, automation, manipulation, or other improper means, such submissions may be disqualified and any associated reward may be withheld or revoked.

7. Verification

ZOZOFIT reserves the right to verify participant eligibility, scan history, account authenticity, survey completion status, and reward delivery information before fulfilling any reward.

Participants may be required to satisfy reasonable verification requests in order to receive a reward.

8. Use of Survey Responses

In connection with this survey, we will collect the following personal information from participants and use it for the purposes described below:

(a) Categories of Personal Information Collected:

• Identifiers such as your name, alias, address, phone numbers, IP address, email address, account name; and
• Commercial information, such as survey data, including information you share when you complete a survey.

(b) Purposes of Use of Personal Information:

• Development and Improvement of Our Services.

In addition to the above, any personal information collected in connection with the Program will be handled in accordance with the applicable ZOZOFIT privacy policy.

9. Third-Party Fulfillment

If ZOZOFIT uses a third-party reward fulfillment provider to deliver rewards, participants may receive reward-related communications, redemption instructions, or support messages through that provider on ZOZOFIT’s behalf.

Participation in the Program constitutes acknowledgment that reward delivery may be facilitated by such a provider. However, ZOZOFIT remains the sponsor of the Program.

10. No Transfer of Rights

Participation in this Program does not create any partnership, agency, employment, or other special relationship between the participant and ZOZOFIT.

11. Program Changes

ZOZOFIT reserves the right to cancel, suspend, modify, or terminate this Program and these Terms & Conditions at any time in its sole discretion.

In the event of any such cancellation, suspension, modification, or termination, unfulfilled rewards may be forfeited to the extent permitted by applicable law.

12. Disqualification and Account Action

ZOZOFIT reserves the right to disqualify any participant or close the account(s) of any user if this Program is used improperly or if these Terms & Conditions, the ZOZOFIT Terms of Use, or any legal requirement are violated.

13. Limitation of Liability

To the maximum extent permitted by law, ZOZOFIT shall not be responsible for any technical malfunctions, system failures, survey access issues, delayed email delivery, or other errors of any kind that may affect participation in the Program or reward fulfillment.

14. Governing Law

These Terms & Conditions shall be governed by the laws applicable to the ZOZOFIT contracting entity administering the Program, without regard to conflict of law principles.
